Please understand that payment of your bill is considered part of your treatment. Should you have any questions regarding any aspect of your financial status with our office, please feel free to contact our billing department at (718) 548-5757. We are participating providers for the following insurances: • Aetna • Blue Cross Blue Shield PPO/HMO • Cigna • GHI • HIP • Medicare • Oxford • PHS • United Healthcare In order for us to properly file an insurance claim on your behalf, we must have a copy of your current insurance card, which must be presented at your first visit. Please notify us of any changes in your insurance or coverage. If your plan requires a referral, it is your responsibility to obtain a proper referral prior to your visit. All co-payments and non-covered services are due at the time of visit. We accept personal checks, Visa, MasterCard, and Discover Card as payment. We do not accept American Express.
