Financial Policy
Thank you for choosing Foot Associates of New York as your health care provider. Please understand that payment of your bill is considered part of your treatment. Should you have any questions regarding any aspect of your financial status with our office, please feel free to contact our billing department at (718) 548-5757.
Our office participates with the following insurance companies:

- Aetna
- Affinity
- Blue Cross
- Cigna
- Fidelis
- GHI
- Healthfirst
- Oxford
- United Health Care
Please call our office to confirm that we participate with the specific plan under your insurance company. You are responsible for deductibles, co-payments, and co-Insurance fees determined by your plan.
In order for us to properly file an insurance claim on your behalf, we must have a copy of your current insurance card, which must be presented at your first visit.
Please notify us of any changes in your insurance or coverage. If your plan requires a referral, it is your responsibility to obtain a proper referral prior to your visit.
All co-payments and non-covered services are due at the time of visit. We accept personal checks, Visa, MasterCard, and Discover Card as payment. We do not accept American Express.
All patients must complete our “Patient Registration Form” and other related forms.
